Employee Life Changes

life change can affect your benefits, pension and other information at McMaster. For your convenience we have included the below information to assist you in this process. Please read below instructions carefully to assist you in updating the information efficiently.

Birth or Adoption 

• For active employees, family additions are made by completing an Extended Health & Dental Positive Enrolment Form and returning the original to your Human Resources Department.  Please note that this form will replace any previous information; therefore all eligible dependents must be listed.  Please find this information and additional information here.

 

Retiring or Leaving McMaster

• How Do I Prepare for Retirement?

• At retirement and thereafter, please ensure that you keep  the Human Resources Department at McMaster University updated with your current home address to ensure you receive any correspondence including your T4 or T4A to the correct address.  If you are a retiree, please complete a Retiree Address Change Notification Form or by contacting the appropriate Human Resources Department.

 

Marriage/Common Law Relationship

• In the case of Marriage or Common Law Declaration, please download the applicable forms to update your information with Human Resources:

◊ For employees participating in McMaster  Contributory Pension Plans and Benefit Plans, please complete: Life Change Package(Marriage or Common Law) - Benefits and  McMaster Contributory Pension Plans 

◊ For employees participating in Benefits Plans only, please complete: Life Change Package (Marriage or Common Law) - Benefits 

◊ For employees not participating in Benefit Plans or McMaster Contributory Pension Plans, please complete: Life Change Package (Marriage or Common Law) - No Benefits or McMaster Contributory Pension Plans 

• If you are enrolled in the Desjardins Group RRSP plan, please also ensure you update your information with Desjardins directly. You can access their forms and contact information here

Divorce or Separation

• Please download the applicable Status Life Change package  to update information with Human Resources:

◊ For employees participating in Benefits Plans only with the University, please complete: Life Change Package (Divorce Seperation) - Benefits 

◊ For employees not particpating in Benefits Plans or McMaster Contributory Pension Plans with the University, please complete: Life Change Package (Divorce Seperation) - No Benefits or McMaster Contributory Pension Plans
