T4 and T4A Slips

T4 and T4A Slips for the 2011 Taxation Year will be sent to Canada Post Feb 29, 2012 to be mailed to your home address

Replacement slips are subject to a $5 administrative fee per slip. This fee is payable through payroll deduction, personal cheque or money order. For payments via payroll deduction (available to current employees only), please provide written authorization in your email request.

Original tax slips returned to McMaster University by Canada Post will be re-directed at no charge if an updated address has been provided to Human Resources.

Issue of replacements for lost slips will begin after March 15, 2012 to allow time for Canada Post returns. Please allow 5 business days for processing your request.

If a replacement is required for any reason, please forward your request to your Human Resources Advisor, along with your current full mailing address.
