Effective October 1, 2007:

Vision Coverage

$250 every 24 months for prescription eyeglasses or contact lenses as a rolling benefit; $85 every 24 months per person for one eye examination.
Hearing Aids $500 per ear every 3 years.
Nursing Up to a maximum of $25,000 per person during any 3 consecutive benefit years.
Wigs Covered if hair loss is a result of chemotherapy only.
Intraoccular Lenses Lenses eligible for all types of surgery, not just cataract.
